S/MIME signature validation requires that the user have the sender's public key.
S/MIME encryption requires that the sender have a copy of the recipient's public key.
2) Does S/MIME even work on a mail-in database with no one particular recipient/reader/owner?
S/MIME encryption would only have been possible if you had given the sender a public key, and all recipients would have had to import the private key. To be honest, it's been too long since I looked into S/MIME in Notes to remember how, or if, that would be possible.
3) Why is the body of the mail readable but not the attached file?
My guess is that the p7m file is probably just there for a signature. I say that because if it were a properly encrypted message and you don't have the key, there's no way you'd see the body. But that leaves a further mystery as to where the PDF file that you expected to find can be found.
